ECG Geek 4 (continued)

Thanks for all the comments on this ECG, Im really having a great time reading them all and seeing the discussion developing.

Some of you have asked for more of the rhythm strip, and it just so happens I have another bit (with a little less artifact on).

Im not saying if anyone has it right or not yet, but I will post the conclusion later tonight.

Heres some more of the strip: (I am trying to work out how to do a ‘click to enlarge’ pic, but in the mean time you can right click and select ‘open in new tab’ for a bigger view)

Have fun and thanks for participating!

  1. Ok this one is much clearer. Now there is a P wave for every QRS and a regular PR interval so that rules out 3rd degree. I dont see any extra P waves so Im not thinking 2nd degree type 2. They do look like low amplitude pacer spikes so im still going with my original guess of paced rhythm but I dont think the pacer is going to last much longer. I think the reason the atropine didnt last long is due to reason as to why he had the pacer placed in the first place.

  2. Lifepack 12s don’t show pacer spikes, it shows up as arrows on the bottom of the rhythm strip.

  3. LP12s usually show pacing as arrows at the bottom, but I have seen pacing spikes, just the other day in fact.

  4. The reason you usually dont see pacer spikes is due to newer pacemakers operating with lower amplitudes. I believe that this gentlemen has a older pace maker.
